I knew he had written liner notes for the CD, but I ended up buying the wrong one -- the Lala Land version with John Takis' liner notes. Would anyone be able to send a scan of his notes?

Looking at the American Heritage Center's holdings online, they only have call sheets back to July 14, 1966. In other words, they appear to have no season one call sheets at all. So, unfortunately, this woman's identity might be an unsolvable mystery.
