A quick check on Wikipedia confirms that it was Post-Crisis (1986) when Alfred was established in the comics as the Wayne family butler. So "The Fear" would appear (rhyme not intended) to be the first time this was established in any media.
In the Golden Age, Alfred's father, Jarvis, butled for the Wayne family, and is was his wish for his son to carry on the tradition of being the Wayne butler upon his death. As there was no Jarvis in the Manor at the start of the Golden Age, I'm assuming Jarvis was relieved of his duties after the Wayne's murder and passed away a short time before Alfred appeared on Bruce's doorstep.
