According to a report in Variety on May 6, 1966: Adam West suffered burns on his face during a "Batman" feature scene in which a "bomb" spluttered too realistically.
Adam does show tan/sunburn lines like that in "The Bat's Kow Tow", but you have to look close - it's about 1/4" below the bottom edge of the cowl. Most visible when he Bat-gasses Catwoman, and again in the closeups when they're in the Batcave together.
