He has the mustache but, why would he be dressed like that on the set in full makup?

It certainly looks like Cesar. My guess is that he was scheduled for makeup at a certain time, but they didn't need him in costume until later. He would have stayed in his street clothes for makeup and only changed into the Joker costume for the shoot. No sense wearing the Joker costume and risking getting it dirty or damaged if the shoot wasn't for several hours.

I remember that picture from Joel Eisner's Bat Book. He looks scarier without the costume.

That is so true! I seem to think I've seen this picture on our "behind the scenes" topic - maybe shot during the 3rd season Catwoman?

That's not from Joel's book it's from Adam West Remembers.
It's an early test shot of Cesar before they decided to go with the wig.

It simply looks like he's enjoying some fresh air before heading to wardrobe and hair. Downtime like this happens on sets all the time.
